Duplicate License/Certificate
Any licensee and/or registrant can request a duplicate wall or wallet certificate to replace one that has been lost, stolen, or mutilated. A duplicate license/certificate may be requested for an address or name change. (If the request is for an address change, name change or the license has been mutilated, the old license must be surrendered to the Licensing Program before a new license will be ordered.)
An "Application for Duplicate Certificate" must be completed, notarized and submitted with the appropriate fee. Please allow 4-6 weeks for receipt of the new certificate.
