Polysomnography

Polysomnographic individuals are registered individuals who may assist with testing preparation and monitoring, documenting observations, data acquisition and scoring, and assist with patient interventions, under the direct supervision of a physician and surgeon.

  • You must mail in the Applicant Declaration, Signature & Notary page of the registration application (page 4), with your photo attached, to the Board at the following address:

NOTE: In order to apply for licensure or check the status of your application online, a Social Security Number (SSN) or Individual Taxpayer Identification Number (ITIN) is required to register with the BreEZe online application system. Professional License Portability and State Registration for Servicemembers and their Spouses

The federal Servicemembers Civil Relief Act (SCRA) authorizes service members or their spouses who currently hold a valid license in good standing in another state, district, or territory to practice in California within the same profession or vocation, if they relocate to California because of military orders.
