The current biennial license renewal fee for a Physician’s and Surgeon’s license is $830. The renewal fee includes a mandatory $25 fee for the Steven M. Thompson Physician Corps Loan Repayment Program, and a mandatory $22 fee for the Controlled Substance Utilization Review and Evaluation System (CURES).
If submitting your renewal online using BreEZe Online Services, you must pay the entire amount required for the renewal. Credit cards can be used through BreEZe Online Services when submitting your renewal.
Note: If you fail to renew your license by the expiration date, your license is considered expired, and you may not engage in the practice of medicine.
Fee Exempt Renewals
If you hold a Military or Voluntary Service status license, you are exempt from paying the $783 renewal fee. However, you are still required to pay the mandatory $25 and $22 fees at the time of renewal. If the mandatory fees are not received prior to the expiration date, the license will be placed in a delinquent status and a pocket license will not be issued.
A physician who holds a retired, inactive, or disabled status license is exempt from paying the $783 renewal fee. However, every physician is still required to pay the $25 mandatory fee at the time of renewal. If the mandatory fee is not received prior to the expiration date, the license will be placed in a delinquent status and a pocket license will not be issued.
If you have not renewed your license within 30 days, following the expiration date, a delinquent fee of 10% of the renewal fee will be added to the amount owed.
If you have not renewed your license within 90 days, following the expiration date, a penalty fee of 50% of the renewal fee will be added to the delinquent and renewal fees already owed.

Steven M. Thompson Loan Repayment Program Fee
The Steven M. Thompson Physician Corps Loan Repayment Program is a statewide scholarship and loan repayment programs for a wide range of health-professions students and recent graduates funded in part through the licensing and renewal fees of various health professionals including the Board (Business and Professions Code (BPC) section 2436.5). Controlled Substance Utilization Review and Evaluation System/Prescription Drug Monitoring Program (CURES) Fee
The CURES allows pre-registered users, including physicians and other licensed health care prescribers, pharmacists, law enforcement, and regulatory boards to access timely patient-controlled substance history information to identify possible prescription drug abuse. Licensees are required to pay the reasonable costs associated with operating and maintaining CURES (BPC section 208).
As required by AB 3330 (Calderon, Chapter 359, Statutes of 2020), the fee charged to maintain the Controlled Substances Utilization Review and Evaluation System (CURES) increased from $12 to $22 for licenses that expire on or after July 1, 2021. The fee will then decrease to $18 for licenses expiring on or after July 1, 2023. Please refer to your renewal notice for the amount due. (BPC section 208).
